1.  Choose what Plan "coverage" you need.

2.  Select a Monthly Contribution "Premium" and Unshared Amount (UA) "Patient Responsibility" you're comfortable with. The higher the UA, the lower the Monthly Contribution. Members are not required to meet more than two UAs within a rolling 12-month period.

How To Choose Your Plan



1.  Let the office know you are a Self-Pay patient and request Self-Pay pricing or discounts

2.  Request an itemized bill for services received

3.  Submit your medical bill into your Summit member portal

       a.  If the bill is under your Unshared Amount (UA), you pay the full amount directly to the provider

       b. If the bill is exceeds your Unshared Amount (UA), you can initiate a Sharing Request where the Summit community pays the portion of the bill that exceeds your UA

The Unshared Amount (UA) is what a member must pay out-of-pocket before the Summit community can cover expenses related to a Sharing Request.
